An Ohio-based vinyl tile manufacturer, is under scrutiny once again for its repeated failure to ensure the safety of its workers. This recent episode has resulted in a staggering $545,853 in federal penalties after investigations by the U.S. Department of Labor (DOL).

Company Faces Penalties Following Report of a Finger Amputation Incident 

The Occupational Safety and Health Administration (OSHA) initiated an inspection at the tile manufacturer after a report of a finger amputation incident in February 2023. A 56-year-old machine operator suffered the injury when their gloved finger became caught in a chain and sprocket system lacking the required safety guards. 

Following the inspections in March and May 2023, OSHA cited the company for several violations, including willful, repeat, serious, and other-than-serious violations related to machine hazards. These violations included inadequate lockout and tagout procedures, inadequate worker training, and lack of machinery guards as mandated by law. 

Businesses Will Be Held Responsible 

This isn’t the first time the tile manufacturer has faced such citations. Similar violations were reported in October 2022, and the company contested the proposed $1.2 million in penalties. Tragically, since 2017, a total of fourteen workers at the plant have suffered injuries, including severe amputations. 

Despite these concerning safety lapses, the company seems to persist in exposing employees to dangerous hazards and allowing them to operate unguarded machines. OSHA’s Regional Administrator Bill Donovan in Chicago emphasized that the tile manufacturer must fulfill its legal obligation to provide a safe and healthy work environment for its employees. 

The parent company, based in Seoul, South Korea, established the U.S. plant in November 2015, where it manufactures vinyl flooring for customers in over 50 countries. The facility employs approximately 200 workers.
