The incident occurred when the worker attempted to adjust a coolant feed line on a tube saw, resulting in his glove becoming caught and his hand being pulled into the saw’s spinning blade, causing the amputation of three fingers. This unfortunate event could have been prevented had the employer followed proper safety protocols and implemented necessary safety guards. 

The manufacturing facility now faces citations for multiple safety violations, including failure to follow lockout-tagout procedures during maintenance, inadequate training for employees in using energy controls on automatic tube cutters, and allowing operation of machines with unguarded components, exposing employees to serious pinch-point hazards.
