Recycled Glass Company Repeatedly Exposes Workers to Hazards 

The investigation, prompted by a complaint on March 9, 2023, revealed a distressing array of safety failures, including nine serious violations, one willful violation, and three repeat violations. These violations encompassed critical lapses such as the absence of required lockout/tagout procedures, exposing employees to fall hazards, failure to enclose sprocket wheels and chains, and allowing unguarded projecting shaft ends. 

This is not the first time the company has faced scrutiny from OSHA. Previous inspections occurred in January 2021, March 2021, and May 2021, indicating a recurring pattern of safety concerns within the organization.

OSHA has proposed hefty penalties amounting to $370,995, underscoring the gravity of the lapses and emphasizing the importance of stringent adherence to workplace safety regulations. Timothy Minor, OSHA Area Director in Texas, stated, “The glass company continues to expose workers to some of the general industry’s leading causes of workplace injuries and death.”
