Roofing Company Fails to Adequately Train Employees in Fall Hazards 

The investigation revealed that the deceased worker and colleagues were engaged in installing metal decking on an industrial roof when the worker fell nearly 20 feet through an opening to a concrete floor below. Shockingly, the company had not provided essential fall protection measures, such as guardrails, safety nets, or personal fall arrest systems, essential safeguards mandated by OSHA’s steel erection standard for such tasks. Moreover, the roofing services company neglected to train its employees adequately to recognize and address fall hazards, a critical oversight that contributed to this incident. 

OSHA’s findings resulted in the issuance of six willful violations, representing each exposed worker, along with one serious violation for failing to provide necessary training. Consequently, the agency has proposed a total penalty of $522,527 for these grave violations.
