Fatality at Wire Drawing Company After Employees Were Willfully Exposed to Unsafe Conditions 

Tragically, the investigation concluded that the fatal injuries suffered by an employee could have been prevented had the company adhered to required workplace safety standards. The incident occurred when the company allowed employees to ride atop an unsecured, site-made forklift attachment to move wire mesh bundles within the plant. Regrettably, during this process, the attachment slid off the forks, causing the bundles to fall and leading to the worker’s fatal injuries. 

The consequences of the safety violations were grave. OSHA issued the wire drawing company a willful citation for failing to provide fall protection for employees working at heights up to 13 feet. Additionally, the company received another willful citation for exposing workers to fall and struck-by hazards by permitting them to ride on improper and unsecured forklift attachments. The agency has proposed a substantial penalty amounting to $299,339 for these violations.
