Eligibility Parameters for Proactive Health Management:
– Minimum of 10 full-time W2 employees. (Proactive Health Management requires 5 enrolled participants)
– If company HQ is domiciled in these states, they are not eligible for Proactive Health Management: WA, OR, ID, NM, CO, MI, NY, MA, NJ, VA
– If Company HQ is domiciled in FL, the group must have 50 full-time employees to be eligible for Proactive Health Management.
